Form #10864 EAMS Stipulation

Hey Mike,

We are having a couple issues with this form, which I think can be fixed by:

Making the Occupation hold more characters (we tried typing in Delivery Driver and it won't hold all the characters), and allowing semi colons and other punctuation in the big text boxes which require more explanation.

The problem with this is that the DWC does not allow punctuation in EAMS forms. Also, the space is limited by DWC and if I increase it then the form is going to get rejected from DWC. I'm not sure what then answer is but it might be best to contact the EAMS help desk via Email for more information.
