Request for QME Panel

In 2013 WCAB forms there is a form, 106a. This is for post 12/31/12 injuries.
In WCAB forms there is form 106, a form revised in February, 2009.
My information is that the Medical Unit will not accept either form for a pre 1/1/13 injury.
However my information is, at the DWC website there is a 2013 form for pre 1/1/13 injuries.
Is this true, and if so, could we have the 2013 106 form?

In A1-Law the legacy form is form 10224

The 2013 form is form 11204. You will also find it when you click the WCAB 2013 forms at the upper left at the A1 Court forms tab.

Michael, your form 11204 is outdated. It is the version that is 106a (1/2013).

The current form is 106 (10/2013). The new form is at: http://www.wcwebzine.com/a-z/qualifiedm ... luator.htm

My staff is going to the DWC website to produce the form, but, of course, we would rather have the form auto populate.

Please make sure you have the latest version. Form 11204 is the 10/2013 version.

I updated A1-Law. I have version 7.2.4c. Then I checked form 11204. It is still the same outdated one, 106a (1/2013). The Medical Unit does not accept an outdated form.

At the Edit Letters tab RIGHT CLICK on the printer icon at the bottom of that window and select Extract a Form Letter and type in 11204 and then test it again.
