EAMS forms

Hi Mike,

Our office, as well as many others, are having constant problems with applications for adjudication being rejected due to inconsistency with the WCAB’s rules. A group of our secretaries recently went to the board for training. Some of the things they learned relate to A1, and they have requested that I relay the following information to you. These are just some small issues that you may or may not be able to address. Just passing the info along.

Regarding WCAB EAMS Adjudication Form….

1) The claims administrator part of the form does not currently autofill, but this is one of the most important pieces of information for the form.
2) Question 5,6,7 are auto checked, but apparently life would be easier if they are automatically left blank.
3) The new EAMS application for the place of injury address has two lines, instead of the old one line of info. Thus the old one line autofill for place of injury address does not work properly.

Regarding Document Cover Sheet…

1) No auto check is needed for the walkthrough question because this box is not needed until the case settles. Apparently the board would prefer both checkboxes to be left blank.
2) Anytime a cover sheet is used, a separate sheet is needed for each date of injury (case#), even though the official WCAB has room for more than one date of injury. Thus autofilling all the dates of injury makes the secretaries have to delete a lot of info.
3) The EAMS names that autofill on application for adjudication are for some reason changed all the time, so the ones in A1 are becoming outdated and thus causing applications to be rejected. Apparently the board suggests solely using there website to get EAMS names.

On another note, the autobackup for A1 that happens with the first person to close A1 seems to be getting slower and slower. Often A1 will say not responding while this is happening.

Re #2 - the cover sheet can remain the same. It is needed for DORs but only separate for Applications. So please do not change the cover sheet format.

Right - for the Cover Sheet often users will change the COLOR of the note or use the UP ON TOP feature so that they can quickly locate it and change it but since one might file several EAMS forms they would not want a cover sheet for every form on one filing as they would a sep sheet.

I'll work on the other items but in the meantime staff can change anything they want on the EAMS form before filing.
