Replacing Typewriter "New Matter Index Set" with an A1 Version?

Dear Michael,
We've been using a "New Matter Index Set" special card (Safeguard Form No. NMS534.. about 5" x 6") that creates a triplicate of specific client information. One copy gets taped into the client file (white copy), another gets put in a "tickle" box for each individual secretary (green copy), and the third copy goes into a main box that is updated when the case is closed/Life-Time Medical/etc (thicker harder yellow file card).
The Card is filled out using a typewriter. However, our office is moving away from using typewriters and would like to create a printed version of the same document.
How would we possibly set this up in A1?
I can see a couple of possible options with A1:
1. the needed documentation pulled automatically from the new client card and printed out x3 on a single sheet (or multiple sheets) of 8 x 11 paper (we could probably tape/glue the information to a harder sheet for our main box 3rd copy.
2. Plus... The Safeguard printing company has let us know that they can create a custom paper setup. They just need to know how it would print properly and the information required.
Would it be best to mail you a copy of one of these New Matter Index Set cards we currently use and figure out how to setup a test document in A1?
Thoughts? Suggestions?
